Memorial Plaque Order Form

It is a Jewish tradition to place a permanent memorial plaque in the sanctuary with the names of loved ones. The plaque is illuminated on the person’s yahrzeit, various holidays and Yom Kippur. Your loved one's name will be published every year in the Book of Life.
